My question is how do I create a form that will build its field list form a query and allow me to link other fields to the form to populate a table of answers.

I have something like a quiz where I have a table of questions and categories. I have a query that returns a result set based on the certain criteria. I need to populate the bottom half of the form with the questions returned from the query and store the user's answers to the questions in a answer table linked to the user and the question.
