I have something like a quiz where I have a table of questions and categories. I have a query that returns a result set based on the certain criteria. I need to populate the bottom half of the form with the questions returned from the query and store the user's answers to the questions in a answer table linked to the user and the question. Then be able to print the test the user took at a later date. Responses will be yes/no and a comment. If you can picture a 5000 question test and the system picks 50 questions to ask the user in a subform based on data collected in the main form.
Create MS Access form fields based on a query result.

Sorry I'm new to this
Sorry, I'm new to this.
My question is how do I create a form that will build its field list form a query and allow me to link other fields to the form to populate a table of answers.
like below
Question _______________ ______ Ans. _____Comments
1) Is the sky blue ______________N o_____It reflects light
2) Is grass green__________ ______yes
The questions come from a question table of 5000 question and I only what to display 25 - 50. The Answers need to be stored with the question reference and linked to the user.
I built the tables, queries and main form but do not know how to build the form for the questions and answers or the best way to store the answers in a table that I can report on.
Thanks for trying to help me, hopefully this will make it easier for you to understand what I need. If not please let me know and I will try to include additional information.Comment

This is a whole heap of complication.
The simple answer is that the wizards will easily guide you through creating a form linked to a query. What's more complicated is handling the saving of that (presumably random) set of questions and answers for logging purposes.
I think in your shoes I'd consider creating a temporary table of the selected questions, including spaces for the answers, and binding the form to that table. When the questions have been completed a process could save the data away to your permanent logging table for you and you are ready to proceed.Comment
