Looking for easy to implement contact us form

Does anyone have a working contact mail form in php with form validation? I recently tried one on w3schools but it doesn't see to work.

or the whole sending script with php validation ?

Easiest way is to use a service like www.emailmeform.com
Don't matter what settings your isp imposes then and it's free. :)
