wanna remove image from title bar

i added an image as icon in the title bar of my website but i wish to remove it.I removed its link,deleted it even clear cookies but yet it's not been removed.
need urgent help

Look for something like this in your code
<img src="abc.jpg" width="500" height="114">

Also it could be in your css code.
