table implement

i have paper form and i needed to implemented to online form but i stuck when on paper form there is table and the user need to fill it i dont know how to put this as online table do i need to change to somthing but what is suitable input that user can easily fill the table online please help me i realy i only have three days left to finish this and its into sections of the form

Note that you shouldn't use tables for layout. Items/elements should be positioned using CSS. See the sticky.
