    Looking for an application that allows a user to fill out a form and then have a mana

    My department is implementing a process improvement idea system. I am looking for an application on which I can set up a form, have employees view and fill out that form, and then have their managers be able to view the submissions and comment on them. Ideally, I would like the employees to also be able to see if their managers have commented or approved their submissions. Is there such an application?

    I was looking at WP Feedback, Survey & Quiz Manager - Pro. This is what I am looking for, but I don't need it to be linked to WordPress. Is it able to be used without WordPress or publishing to a blog? i.e. sending out a link internally to employees.

    It sounds like you have a pretty custom requirement and may need to implement your own web application for your specific requirements.

    It seems like a simple web application to develop:
    • The ability to log in as users (you could manually add users into a database if you don't want to manage users through this application)
    • One form to submit ideas
    • A page that displays the submitted idea and has a "comments" form that appends to the original idea
    • A page that allows people to search for ideas
    • Perhaps a page that lets people manage ideas (set them as completed, delete them, or allow people to edit the content of the idea)


    The database for this seems like it will be a pretty simple design too.

    Have you considered creating your own custom application using a web technology like php, asp.net or something?

      Yeah, that's what I was afraid of. :) I'm not a pro at programming (I was a ChemE in school), but I am fairly quick at picking these things up. I just wanted to make sure that I couldn't find one already in existence to save time. I'm going to attempt to do it on my own.
