Generating PDF from Form

I have a form and I need to be able to convert it to a PDF.
By this I mean the form basically looks like what i need the PDF to look like.

How does one go about doing this?

If you google for pdf printer download, you will find freeware that installs/looks like a printer, but creates a pdf file instead of a printout. So, you can screen capture the form into paint and then "print" it to a pdf. I think that I have CutePDF.
