SQL Server 2000 Service Pack 3a installation fails

I have an installation of SQL Server 2000. Both MBSA and @@version
show this to be the gold release. When I install Service Pack 3a, it
shows no error. But after restart of the server, it's still unpatched,
    by both MBSA and @@version.

Any idea what may be going on here?

Do you happen to have several instances of SQL Server installed on the
    machine?

    (And what is MBSA?)

      Erland,

MBSA = Microsoft Baseline Security Analyzer

I think there's only one instance. From Enterprise Manager, when I
select 'New SQL Server registration', only the one instance shows up.

Better to use the SQL Service Manager, or open Computer Management
        and check Services.
