Need Help on user registration form

Hi every body,,,
I have to make php scripts with mysql for prepaid customers.
Problem is i want assiatnce...... ..
i have two fields in html form one is Activation Code and
the other is Access Code, How to compare these with mysql
table to verify the codes.and then to go to registration page.
where user enter their username,passwo rd,confirm password,
fullname,Ic number,email for confirmation(An email sent to his email add
for confirmation link).
If any body have some idea give share with.
Plz give details if have.

So what code have you developed yourself until now in this case? Please show it to us, so we can help you when you are stuck somewhere.
