Hi

Is there any possible to create a pdf file at runtime with textboxes
and checkboxes .
The output pdf form looks like a application form and the user can
fill the text box and check boxes and can printout the form.

look like

Name --------------------
Gender checkbox checkbox
address ------------------------------------
