scripting for forms

I am making a simple form but wish to use something other than the "text" response script on occasion. I would like to use drop down menus and check boxes. The trouble is that I have no idea where to look for examples of these scripts.

I can write up the form in html with no problem but don't know how to make the mailprocessor.c fm see the different entry systems. Does anyone know of (and there must be some out there) web pages with script examples already set up for something this simple?
