Switchboard form opens blank form

We have a switchboard form that displays buttons based on a Switchboard table. The button opens forms based on data from Switchboard table, but opens the form with Data Mode=Add. When users click on the button, form opens empty/blank.

Any clues on how to fix this issue?

What is the behaviour supposed to be? Let's concentrate on that, rather than on what is going wrong.

You have

1. Switchboard Form
2. Press Button X
3. Switchboard Creates Form X
4. Switchboard Sets Form X to Modal
5. Form X Displays in Add-Record Mode

What else should happen?

That's how it should open. If you're opening it in Add mode, why would there be any data? You haven't told it what data to add yet.
