access report in the format

i have the table with the format
empcode,amount, date
with data
11,40,1-3-12
11,60,5-8-12
12,20,1-6-12
and so on
i want to make a report in the format

ecode jan feb march apr may june july aug sept oct nov dec
11              40                     60
12                            20
13    80                                          70
14        10

Kindly help by providing most suitable suggestion
