deploy my MSSQL database to site domain

I develop ASP.NET website using VWD and SQLserver 2005 when I try to deploy my web site I dont know the connection string information of the server (database name ,username ,password,serve r name)and I want to kow if this information is my job or its the job of my consult provider job and if it is my job how can I create MSSQL database in control panel of the site domain
help me please and I thank you very much

From the wording of your question, I would guess your ISP should know the server connection details and possibly generate the database name. But the username and password of the database should be set by you.
This all depends on your provider and the service you are paying for.
These guys are usually helpful.
