php and mysql - insert and read files

Can someone help out on how to insert file into a folder and its path in the database (Mysql). And then how to retrieve the file?. THE FILES INCLUDE JOURNALS, BOOKS, RESEARCH. help, please.

And, if not, can you please rephrase the question? I'm a bit confused by the words: "insert, into, its, in" withing one sentence... ;)

And could you say more about:
