How to email details of a form in a pdf attachment

How can I insert into Mysql and at the same time create a pdf file that is instantly send out as an attachment to a specific email ?

1. mysql operation in php
2. how to create PDF using PHP, I will suggest you to look at TCPDF, MPDF, and so on.
3. Look at emailing using php.
4. Look at MIME type email sending.
5. look at Base 64 encoding
