What is the definitive method of install PHP as a CGI on Apache 2.2 in Windows

I have been going around in circles trying to install PHP as a CGI on
Windows.
Is there a definitive document somewhere on that?
Even the php.net website is rather vague.

Under what HTTP server, Apache 1.3.*, Apache 2.*, or IIS?
