IPersistStreamInit pPSI ->Load(pStm); returns a 0xc00ce584 hr

I have an xml file that was written as image data into a database column.
When I call load() an error hr result is returned that I cannot locate
anywhere.

I am using XML 60.

Any ideas what this error condition is?
