How to upload file from a file input element in a html form?

I am new to Javascript/AJAX.

I need to upload a file to a server using a file input element(using the 'onchange' event). I need to write a javascript/ajax code for the same.

I would be very grateful if someone amongst you might drop a little hint in doing so.
