How to pass parameter name with url of previous page

I have one form on that i used netbeans one line code for sending mails to different addresses. that mail address i am getting from the table mysql. that mail address field i am getting in while loop. now alongwith this i need to send one parameter alongwith url. and catch that field base report on report form.

how to do pl. help.
