Form Creating Help

Dear,
i have a table where following fields are exist.
Slip No = Auto Number
Total Slip = Total no of Slip of an employee
Code = Employee Code
Name = Employee Name

i want to create a form to enter data in this table where slip no will be auto show,Name will show auto look from another table, total slips will be shown auto on entering Employee Code and also have a button to preview a report.
