To retrieve value from a table and display it on the FORM

Hello,

I am not sure whether I have asked this before but I havent resolved his issue yet. any kind of quick help will be appreciated.

Requirement:
To get the value of a field from a table on the basis of three other fields (will be put in the combo boxes on the form) and display it back on the FORM textbox and radio buttons.
Example:
User put Grade, School, Teacher on the screen. And I have to retrieve the particular question and display in the textbox on the FORM and the four solutions for the question should be displayed on the radio buttons on the FORM.

No Success yet. have tried quite a few stuff.
Currenlty trying DLOOKUP. No luck in this yet.

Help appreciated.

You know, I've read your post a few times and I still can't figure out what you're asking. How about supplying a bit more information on what you are trying to do and an example of where you are currently at with your project.
