    Form Searching

    Hi,
    I've got a form which logs various issues.

    What I'm after is a blank version of this form where I can input data.
    Then hit a button and it finds all records with that data.

    I remeber doing this at uni a few yrs ago - but typically, in the
    workplace I've forgotten pretty much everything.
    Any help would be greatly appreciated.
